  • Please Note: Before you can submit a request to have your lecture recorded you must ensure your course shell is set up and activated on Quercus. Academic Administrative Services can assist you with your Quercus course if you require help.

    You can contact them at: academicadmin@dentistry.utoronto.ca If you are planning to record all of your lectures for the academic year, please include a copy of your lecture schedule and be sure to indicate the start/end times and room numbers for each lecture

  • Purpose

    This form is to be signed by the person (“the Presenter”) who has agreed to be recorded, in both audio and video formats, as the principal party to, or as part of, a lecture carried out within the Faculty of Dentistry at University of Toronto (“the Faculty”). The purpose of this form is to seek consent for the videos and/or recordings to be taken and subsequently to be used in a number of media, including the Internet, audio files and video files by the Faculty. The Faculty in turn offers a commitment that said recordings will only be used for Faculty programs. Any additional use will require written consent of the Lecturer and the Dean of the Faculty of Dentistry, University of Toronto. Information and Instructional Technology Services

    The Presenter

    I, the undersigned, agree to my lecture being recorded in video and audio formats by the Faculty of Dentistry, University of Toronto. Where a recording is being made, I will notify everyone present that a recording is being made.

    I confirm that where material is included in the recording which is the intellectual property, including copyright, of another party, I have permission to include the materials in my lecture.

    The Faculty

    The Faculty of Dentistry agrees to only use the recordings for educational purposes within Faculty programs and will delete the recordings on request.

    Additional considerations - students/non-employees

    In order to make it clear to all students and other parties attending the lecture that a recording is taking place and to allow them the opportunity to opt out of the recording, the lecturer may provide the following statement:

    "This lecture is being recorded in video and audio formats by the Faculty of Dentistry, University of Toronto. The recordings will be used for educational purposes and will include conversion to digital formats which may be stored for future access and distribution. Any person who does not wish to appear in the recording should not stand near the podium or microphones during the presentation"
